If you are considering a concierge style or subscription medical practice, here is the experience of one patient at an MDVIP practice in NWA.  If you have already joined one, and are not seeing the promised results, then this report may also be of interest to you. There may be lessons to be learned from reading this story. In hindsight, this patient admits that she was remiss in not doing appropriate research before joining the practice, believing only what she heard from the seller, her doctor, Dr. JB.  Had the patient known what is presented here, the decision to join would not have been made.

We all are responsible for our own actions, but we all deserve full disclosure before making expensive choices.   Because others may benefit from seeing this information, the decision was made to share this report online for others to see, based on the scripture principals found in Ez. 33, 1-6.   The snapshots were collected from public websites and research was done in the Summer of 2019.

We trust that you will be enlightened and able to make more informed decisions about joining or staying with a concierge medical practice.

This report was prepared by long-time acquaintances of the patient whose experiences with an MDVIP affiliated practice are used in the report as an example.  Having worked together over the years since the Internet came to be, and having seen the results of the Internet's ability to make missing information available, we trust that this report proves to be useful to other patients who might be searching for the pros and cons of an MDVIP or other concierge-type membership.

 

 

 


Recent Research regarding MDVIP:  
This may be only the Tip of the Iceberg?

Early in 2019, the sample patient began receiving emails each month from MDVIP, to sign an updated membership agreement, using an on-line "DocuSign" system.  The patient did not sign, as the email did not provide links to read the updated agreement.  This is one such email.


Following some research at MDVIP's website, the Terms & Conditions of the updated membership agreement were found by the patient and reviewed.  We include an excerpt (highlighted area below) of what the patient considered to be a red flag

Dr. JB said in a discussion with the patient  that he had researched MDVIP for 5 years prior to signing on with them, and that they had NEVER had an issue with one of their physicians...really??

To gauge whether others may not be so happy with MDVIP, a check was made of MDVIP's local Better Business Bureau in early August, 2019, which showed 18 active complaints and the notice that MDVIP is NOT accredited by the Better Business Bureau.

But first we'll review and examine the promises that were made
and the promises that were broken.

 

 

 

 

The Promises Made from the Doctors [and MDVIP] to the Patients.

During oral sales pitches within DR's office visits and at both of the 2016 Embassy Suites Presentations [where members were actively recruited], they heard these promised benefits of MDVIP membership:

We would have the advantage of getting to know our fellow members, through group gatherings, or seminars.
(Never happened)

The patient load of OUR Dr could be no greater than 600 so that much more time would be available per visit.
(The more the visits, the more the doctor can bill insurance)

If we were to travel in the US, medical service would be provided by other MDVIP Doctors.

We would have access to OUR Dr directly by phone, email, or text even after hours.

We would be able to see OUR Dr on same day or next day, if needed.

As members we could reach OUR Dr directly when needed.

Membership was to be $1650 per year per patient.

Pay Annually, semi-annual, or quarterly.

Service would be available 24/7.

MDVIP would do the billing.

PLUS ...the last and best promise ... We would each receive a unique and unusual set of Annual Wellness Lab Tests that would be done at the highly touted Cleveland Heart Lab (located in the Cleveland Clinic), like none we had ever received before.  Note:  This same Cleveland Heart Lab is now owned by Quest Diagnostics.  The online article announcing this acquisition was dated Nov.27,2017.

Broken Promises Experienced by the Patient and The Disturbing Trend

During all the time as a member of MDVIP, since Platinum Health opened their office in mid 2016, almost all of the patient's dozens of calls to Dr. JB's cell number went to his voice mail, except for two, in which he said he would call back that evening...he did NOT call.  When asked days later during an office  visit, why he didn't call back, many excuses were offered, and instructions given the patient; "the next time, be sure to leave a message that this was the second or third call and it was urgent to talk to him"!    Dr. JB NEVER responded to any of the patients many emails.  Text messages were not tried by this patient.  As to the promise of getting to know others in the MDVIP community of patients, there have  NEVER been any group meetings or seminars during the entire period since the patient joined MDVIP in mid 2016, until the time of this report, Summer, 2019.  During several office visits, this patient had asked if Dr. JB's experience with MDVIP was positive, he replied that he was very happy with the way the practice was going.

The fact that Dr. JB  would begin leaving the US (and all his patients) where he could NOT take patient calls, was even more disturbing, and this happened at least 5 times that the patient was aware of.  The way that the patient was notified in advance of the impending lack of his presence, was by automated phone message from Dr. JB, either played directly upon the patient's answering of the phone, or the message was left as voice mail.  The message would say something to the effect that "I will be out of the country between these dates, and NOT able to be contacted; if you need refills, do such and such; if you need service while I'm gone, call the office during regular hours and see my partner".  According to the patient, this notification by phone has happened a total 5 times.  On one of these occasions, the trip was postponed, but the patient was not notified that Dr. JB was available during the dates that had been blacked out by an automated call.  All these messages came on short notice prior to date of departure.  When the patient inquired of MDVIP as to whether a refund for lack of promised service, would be applied to the payment due, the answer was "MDVIP does NOT give refunds".

The patient logged these service blacked-out periods, as they were received via Dr. JB's recorded phone message as follows:
 
In 2017: #1:  Feb. 25th to Mar. 4th with an ending weekend; total of 10days.
    #2:  May 26th to June 2nd with an ending weekend; total 11 days.

In 2018: Jan. 5th - Jan. 13th with an ending Sunday; total of 10 days.

In 2019: (as of July 31st) #1:  Feb. 27th - Mar. 11th; total of 13 days. Trip was later rescheduled but NO notification was given to patient to cancel the blacked-out days so the patient was subjected to the stress of being without access to Dr. JB the entire time.
# 2:  Jun 3rd - Jun 17th with an ending weekend; total of 17 days.

  When the patient visited Dr. JB after each of these black-out periods, questions about the reason for his absences revealed: Dr. JB and wife took a vacation to Mexico, next a trip to the Orient, then a vacation to Ireland,  then a trip to Africa to dig water wells, which was rescheduled at the last minute for later in the year!  These absences seem to the patient to be blatant breaches of the promises that he and MDVIP had made,  When pressed, Dr. JB told her that his taking patient calls all the time was not promised in the agreement; "it was only a courtesy!"  He insisted that all of the money paid by the subscribers went toward the payment to Cleveland Heart Lab for their annual lab services. 

His justification that his patients were left in the hands of his partner, who had never seen the patient before, and whose practice philosophy is grossly different than Dr. JB, did not give comfort about this violation of the the mutual agreement that we all had with him.  The patient did inform Dr. JB that she would be doing more research on MDVIP to evaluate whether she was "getting what she paid for" and whether to continue as a member.

 

VASAYO and Dr. JB

In mid-2019 the patient became aware of the attachment and association of Dr. JB and his wife with a Multi-Level Marketing organization, VASAYO, whose product brochures were being offered to patients during office visits, with directions to call his wife's cell number to place an order. 

In the Spring of 2019, Dr. JB had even suggested certain VASAYO products to the patient, without disclosing anything except that he was taking one or two of them himself. These are the distributed VASAYO flyers received by the patient during an office visit.

Then the patient learned that outside meetings for VASAYO product buyers were being held at Dr. JB's home, where they became distributors!  This unexpected and predatory use of his patient population for personal gain is both disappointing and revealing about the current motivations of Dr. JB.  When Dr. JB was questioned about this alliance with Vasayo, he bragged that he and 3 other doctors were in charge of Vasayo's pod-casts where Vasayo distributors could be better trained.  Seriously??  To the patient this was undoubtedly a conflict of interest! 

 

From the authors of this report...A physician should have the best interest of his PATIENTS at heart...not his OWN best interests.  He should speak the truth and keep his word.   Anything less than that is a betrayal of the patient's trust. 

 

 

 

 

 

 

Patient's Discussion with Dr.JB regarding  MDVIP and Vasayo

When the patient explained that she was trying to decide whether or not to continue with MDVIP, but  that she would wish, however, to keep Dr. JB as her primary care physician due to their 12 year-long association, Dr.JB responded that he could not treat her without her continued payment of his fees (which upon later reflection, felt a lot to her like Extortion).  When asked to explain why, Dr. JB said is was "too complicated" to explain; but it was a legal issue.  Since the patient was part of a  PPO Medicare Advantage plan, she could go to any physician in their network without a referral, which includes Dr. JB .  His response to her question about how he could refuse her, was patronizing, to say the least.  It seems that it may be all about the money. 

When the patient asked Dr. JB about his association with a multi-level marketing company, VASAYO, whose products he had recommended, and whose brochures he had handed out to the patient during an office visit, he admitted that he is affiliated with Vasayo, also.  It seems that he not only sees MDVIP patients for a fee, but also, as a representative of Vasayo who can then recommend their products, he has the opportunity to turn their purchases into revenue for himself, and then further to possibly turn those MDVIP  patients into Vasayo distributors, also generating more revenue for himself since he is higher on the pyramid.  This realization was chilling.  A whole other side of Dr. JB was revealed.  Anyone involved with Vasayo should do their own research on the questionable history of its founder with his last MLM company, Mona Vie, and not rely on what Dr. JB has told them.  We discovered a very interesting online article summarizing this history from a Florida law firm (the same state in which MDVIP is located)  entitled:  Vasayo: MLM's latest "magic pill" or another pyramid?

In order to keep accurate notes and a medical log of visits, this patient has used various pocket recorders over the years during every office visit with Dr. JB from 2007 when they first met;  he will  probably not remember that his permission was given, but it was.  One noticeable and consistent trait of Dr. JB, is not to remember what was talked about and covered during the last visit, frequently asking the same questions at every visit and performing the same routine exams.  Another consistent thing was that Dr. JB always remembered to take every opportunity for self promotion.    As part of the background for this page, the patient made available to those involved in researching the presented material for this report, an audio section of her dialog with Dr. JB concerning his involvement with MDVIP and Vasayo as a confirmation of what had transpired between them.  Although the audio quality is less than perfect, we included it below.

Depending upon your available bandwidth, it may be best to allow the file to download first, to provide the best audio clarity.]  When you listen, it seems clear that Dr.JB certainly has rationalized and justified his behavior in his own mind.    As many doctors feel exalted above their patients, Dr. JB seems to be no exception.   In that light, it seems Dr. JB may have joined the predatory ranks of industrialized medicine, run as a business, rather than as a calling to serve his fellow man.  

 

 

 

Following the Money...

MDVIP provides it's "consulting " services, bills the membership fees to all patients and then does the collection and disbursement of money to the physician.

Based on the patient's discussion with Dr. JB in late July of 2019, there were, at that time, 970 MDVIP physicians; and MDVIP takes a third of every membership dollar as a commission for their services.

Using the lowest membership charged, a sample rate of $1650 per year (some practices charge up to $2200) for a maximum of 600 patients per physician for 970 physicians, and using a method remembered from college days of slide rule usage, we did the math, and you can too.

MULTIPLYING...

...we are talking about a potential 960 million, 3 hundred thousand dollars per year, handled by MDVIP for all of their physicians. (If some of those physicians are charging more than $1650 per patient, then that total can easily be over one Billion dollars per year coming into MDVIP.)

But staying with our sample base-rate calculation, MDVIP's hefty commission would potentially amount to $320,100,000; that is 320 million 100 thousand dollars per year into the MDVIP pocket generated by their 970 physicians, with potentially $640,200,000; that is 640 million, 200 hundred thousand dollars total per year remaining to be distributed among 970 physicians — which computes to an average  potential share of $660,330; that is 660 thousand, 330 dollars per physician per year!! 

Should the MDVIP subscribers continue to enable their doctor's MDVIP revenue when that doctor is NOT meeting their expectations or NOT following the promises made to the patient?  Are the MDVIP subscribers being held captive in fear with a "gun to the head", if they consider leaving?  Has their doctor kept his promises to the patient?  Has there been a breach of contract?   Has there been a breach of trust?  These are all questions that each individual has to answer for themselves.

What can the patients do?    We'll discuss that next.

 

 

 

  

ADVICE from the SAMPLE PATIENT'S INSURANCE CARRIER

Our sample patient, after receiving the figures in the above Follow the Money section, was amazed. It is no wonder the MDVIP doctors are happy.  If the doctor can minimize the amount going to Cleveland Heart Lab, then he gets to keep the rest!

Other questions arose.    Do Medical insurance  and Medical Advantage Providers, that are also covering patients who are part of an MDVIP practice, allow this extra charging of their patients?  Do they even know about it?  Do they know that their network-contracted doctors are threatening patients with withdrawal of service unless the patients pay them extra fees? 

A call to the patient's insurance carrier's customer care service could not answer the question.  The query was bumped up to the next level.   Soon, another agent called the patient, listened to the background and offered to do further research .

Later the agent called back; advice included; if a patient is considering leaving the MDVIP practice, they should not be rushed into anything; the patient should ask around, see other doctors, since the patient's plan was a PPO.  (Our sample patient put the matter to prayer, and was led to discover that a recent and very competent Integrated Medicine physician had moved into the area as a replacement for a  retiring, long-time physician at the Rogers Family Practice [NW Family Care Rogers] which was also part of her network.  A sample visit proved to be an answer to prayer.) 

The agent further advised that patients should, at some point, directly ask their MDVIP doctor to continue as their primary care provider, even if they decide not to be a  part of MDVIP.  If the doctor refuses, patients should report them to their own medical insurance or Medicare Advantage insurance carrier. That is something constructive that can be done by patients to help expose this wrongful gun-to-the-head treatment of patients.

The agent also explained that contracted doctors are not allowed to double bill; in other words, no charges from your MDVIP annual labs and exam should be shifted to Medicare, as the patient has already paid for all Cleveland Heart Lab associated services by paying their MDVIP fees; their payments are supposed to be conveyed by the MDVIP doctor to Cleveland Heart Lab using the patients' MDVIP fees.    Patients are advised to check their Explanation of Benefits or monthly statements of medical/hospital services to make sure that nothing from Cleveland Heart Lab was billed at any time to the patient's medical insurance. If double billing seems to have occurred in any of the years that you have been with MDVIP, then report it to your insurance carrier.

 

 

 

 

From the authors of this report ... Industrialized medical care, which is now considered only as a business,  has a different set of standards where patients are just resources to be harvested, "sheep to be fleeced", targeted for the perpetual revenue that they can generate and the predictable profits that will continue until the patient either has no more resources or the patient is dead.  In this diabolical medical model, the patient's intended lack of recovery to a state of health is just the collateral damage perpetrated with malice aforethought.  Nothing makes money like a permanent patient! 

Let us all pray that each mercenary, hubris-filled, self-serving, predatory doctor will be convicted of their sins, acknowledge them, confess them, and enter into repentance.  We shall all stand before that same judgement seat of Christ, where our deeds done in the flesh will be replayed, and recompensed. 

 

 

 

 

Further thoughts on the spiritual aspects of the requirements of doctors...

All doctors have their own story, their preferences, and their opinions, which they share one-sided with their patients— sometimes over and over again. God gives the doctors the stewardship over those patients who come to them, and the doctors will answer to God.  Much responsibility rests upon the doctors, and each doctor makes decisions and chooses directions for those patients.  If they (the doctors) are not faithful in their duties, that will be revealed at their judgment; if a doctor comes to himself and realizes his error, he will be shown what he must do. 

Our sample patient remembers that Dr. JB bragged to her on numerous occasions of his close association with certain prominent pastors in the community, who are also his patients.  Perhaps he might find direction and meaning from their counsel at this time, as to how these included scriptures below, apply to all of us, to teachers, to pastors, and even to doctors.

February 28, 2020 — Local Update to the MDVIP Issues Report:


On January 30th, 2020, 4 months after deciding to leave Dr. JB's practice, our sample patient received a mysterious envelope, addressed by hand, delivered to her home, via USPS Certified Mail, with a rubber stamped return address of Platinum Health & Wellness.  Thinking that perhaps Dr. JB had thought better of not keeping his part of the bargain, she hoped that the envelope might contain his apology and the refund that she had requested, in the form of a cashier's check.  {She had previously asked the MDVIP Franchise Headquarters as well as Dr. JB for her money to be refunded because he failed to perform.  MDVIP said, No, outright and  Dr. JB had shaken his head and launched into a rehearsed list of excuses.  These extra MDVIP patient fees, totaling approximately $5400 were paid by our sample patient, on-schedule, with the expectation that Dr. JB would perform ALL of the list of MDVIP promised services.  Dr. JB got his extra money from her, on time, as promised;   {According to the sample patient... she kept her side of the deal, but Dr. JB did not.} 

However, upon opening the envelope, our patient found an intimidating demand letter signed only by Dr. JB, [with a copy to an attorney].   The letter included State of AR civil and criminal legal citations.  It seems that our sample patient, who is a now a widow in her 80th year, is being directly intimidated, coerced and threatened with legal action if this MDVIP Report, [which was first researched and written in the Summer of  2019], continues to stay on-line past February 28, 2020. 

This frightening attempt to blackmail an elder patient seems hard to believe.  The letter signed by Dr. JB  claims that he never gave permission to record her office visits; the letter further claims that he has sustained financial damages from the release of this report, and also claims that the financial viability of his practice is being threatened. We would ask...how can the loss of a future revenue stream from this one sample patient threaten the financial viability of his practice?  

So logically, we might assume that there must be other patients who have decided to leave his practice for their own reasons. Dr. JB now appears to be very unhappy about any loss of patients, and is trying to cover his future revenue stream, by demanding that this report be removed from internet free access to other potential MDVIP patients, who may be researching the pros and cons of being an MDVIP member.  His letter also demands that all copies. transcripts, summaries, or other records derived from the posted recording and all other recordings of our conversations be destroyed by our sample patient!  But hold on, patients are not possessions to which any doctor is entitled.  They are human beings, free agents who are  capable of deciding to move on.  We suggest that Dr. JB is trying to find someone else to blame for what he may have brought upon himself.   

As to Dr. JB's claim that he never gave permission for the recording of office visits:  Our sample patient with her husband (a retired medical professional) recalled this discussion with Dr. JB when they first interviewed him, as potential new patients on Sept. 25, 2007, about their ongoing expectations, if they were to choose him as their Primary Care Provider.  The discussion went, as follows:    Because older patients often do not accurately remember what transpires in an office visit, the doctor's instructions are forgotten, and important information is lost by the time they get home.  If the patient attempts to take accurate notes during the visit, it is stressful and distracting to the patient, often lengthens the visit, annoys the doctor, and the take-home result is no better.  Therefore Dr JB was told that, as his patients, they (Dr. & Mrs. sample patients) would be recording each and every visit, so that they could take in as much as possible of the doctor's counsel in real time without pressure, and then they would fill in the gaps of the details later, at home, as they logged the recorded visit; they would verify with each other the summary before it was committed to the file.  At that time, Dr JB's response was very complimentary and approving; something to the effect that he completely agreed, and thought it was such a good idea to do that; he wished that more patients would do likewise; and he might recommend it to others.  The results for Dr. JB were two more patients to generate revenue. They were his patients beginning in 2007.

At that time (2007), Dr. JB and his current partner were providers employed by Washington Regional at Hidden Springs in Rogers.  Following that, in 2013, Dr. JB and his current partner both became providers for Northwest Medical at Sugar Creek in Bella Vista.  It was during that period, in the fall of 2015, that our sample patient became a widow.  She continued seeing Dr.JB as he began telling patients that he was going out on his own in 2016, into a concierge practice with a maximum of 600 patients.  His patient numbers were in the 4 figures at Northwest Medical, and he needed to recruit enough of those patients to follow him to his new practice — patients with enough money to pay him the concierge fees that he required. 

The opening of their new franchised MDVIP medical practice happened in 2016, and was named Platinum Health and Wellness which is still operating in the HOG Cancer Center.  Our sample patient believed Dr. JB's promises and followed him into the MDVIP practice where she soon began to notice a lack of performance as to her specific requests and needs. She stayed on until the Fall of 2019, when her last quarterly payment expired with no resolution to her complaints and continued admonitions of Dr. JB for not keeping his word to her.  As mentioned earlier, she was refused further service by  Dr. JB, unless she kept making her MDVIP extra payments.

In response to the demand letter of Dr. JB, the patient further advises that I will be keeping in a secured location, all family medical records and logs concerning my deceased husband's and my time with Dr. JB, since it has now been made clear that this report clearly shows examples of his lies. And, as to the recording of his easily recognizable voice, which  verifies the truth of the material shared in this report, I will be keeping my copy as well.   No wonder Dr. JB wants all evidence destroyed!  For a doctor to not give full disclosure to his patients, and try to cover up the facts that might strongly impact their opinion of his integrity, honesty, and motivation, is very interesting indeed. 

But back to this update...Since the initial release of this free-access on-line MDVIP Review and the accompanying research in late summer of 2019, the electronic files have been kept on a remote server (nowadays called a Cloud server).  It is most likely that the information in this report has been viewed, printed, bookmarked, and copied by any and all viewers.   Furthermore this information would typically have been crawled by Internet Spiders, mirrored for various search engines and even archived in web indexes.    Therefore, Dr JB's demand for removal makes no sense, as it will not reverse the effects that Dr. JB's own choices and his own actions have unleashed upon his own concierge practice.

All patients have a right to tell their story and have an equal right to their own opinions.  Their medical records are their own private property including the lab results, X-rays, reports of findings from whatever is done to them, personal summaries that were kept of each doctor visit. What if those records provide evidence of medical errors or fraud?  We would ask...to whose advantage would it be for those records to be destroyed?   So what is Dr. JB's true motivation for these demands? 

Upon further research , there is one last topic that we need to share for this update... that of Elder Abuse and Neglect which is rampant in today's world; we all have heard the horror stories and it is often about taking money from elders by deliberate, premeditated methods.  Here is more information that may be useful to elderly patients or their caregivers, and we felt led to draw your attention to the following...

There is a category within the subject of Elder Abuse entitled, Healthcare Fraud and Abuse, which is — Carried out by unethical doctors, nurses, hospital personnel, and other professional care providers,
that can include:
1] not providing healthcare, but charging for it;
2] overcharging or double billing for medical care or services;
3] getting kickbacks for prescribing certain drugs;
4] over medicating or under medicating;
5] recommending fraudulent remedies for illnesses or other medical conditions;
6] Medicare or Medicaid fraud. 

Dear Patients,  If you recognize any of these abuses in your provider, it would be wise to detach yourself from him/her and find another provider.   Your life could depend upon it.
